-Provide current immunization record and all relevant medical information

-Provide current insurance information

There are forms you must fill out prior to being seen the first time. You may fill them out online, get them emailed to you, or stop by our office to pick them up. 

For any acute illnesses, please call our office if you feel you need to be seen.

It is your responsibility to have a current insurance card available and to be familiar with your own policy. We will submit the claim to your insurance company. You may be responsible for any portion of your visit that's not covered. Payment is expected at the time of service. We accept cash, check, or all major credit cards.
